Job type: Full time (38 hours per week)

Main Duties and Responsibilities

  • Develop the Pre Assessment service with the support of the Lead Nurse
  • Will be responsible and accountable for the delivery of safe and effective care within the pre assessment environment
  • Responsible for the initial clinical contact to all our patients to ensure they are safe on their journey by ensure guidelines such as anticoagulation, diabetes and relevant comorbidities are known and dealt with appropriately and safely for the community setting.
  • Take an In-depth history from the patient and evaluate previously unknown health issues
  • Be expected to work independently and autonomously
  • Ensure excellent verbal and written communication skills to ensure that information is disseminated to patients and relatives
  • Ensure JAG guidelines are upheld and adhered to within pre assessment
  • Triage of referrals into service ensuring they meet contract specifications.
  • To support a culture of continuous quality improvement, evaluating standards of care and leading in the implementation of changes and nursing development.
  • Support the clinical team in the development, maintenance and review of nursing care pathways.
  • Supervision, teaching/mentoring of nursing assistants, arrangement of training and mandatory study days.
  • Maintain a Personal Professional Portfolio in accordance with the requirements of all professional bodies
  • To produce and maintain accurate clinical records of treatments and procedures.
  • Maintenance and review of health promotion literature and information for patients and ensure that any changes are reviewed at our Clinical Governance meetings.
  • Liaise with other primary care and secondary care colleagues to ensure effective communication and share best practice, ensuring the clinical team operates in accordance with current guidelines, including NICE and NHSE.
